Carole Michalski-Monnerat, PhD(c)

Carole Michalski-Monnerat is a BSN, MScNS (2019) and current PhD candidate in the Institute of Higher Education and Research in Healthcare (IUFRS) at the University of Lausanne. She works as a Clinical Nurse Specialist in the care department of internal medicine at the Neuchâtel hospital network (RHNe), with ten years experience working with adults with multiple chronic conditions.

Her doctoral thesis aims to co-develop a new model of transitional care from hospital to home, led by Advanced Practice Nurses : the INSTEAD project (Infirmière de pratique avaNcée pour des Soins de Transition Entre hôpitAl et le Domicile). To ensure that the components of the transitional care model fit with contextual needs, the INSTEAD project identified the priority patient population to target and is identifying patients, relatives and healthcare providers’ needs regarding hospital-to-home transition. An advisory committee including key stakeholders (patients, relatives and providers) is engaged to make recommendations based on the project’s results.

Research interests: Advanced Practice Nurses development, implementation and evaluation, continuity of care between hospital to home and key stakeholders’ engagement to co-design research and clinical projects.
