Sa th\u00e8se a \u00e9t\u00e9 effectu\u00e9e sous la direction du Professeur Pomini, dont elle a \u00e9t\u00e9 l\u2019assistante durant six ans. Sa recherche a eu lieu en partenariat avec le Service de Psychiatrie Communautaire du Centre Hospitalier Universitaire Vaudois (CHUV), o\u00f9 elle exer\u00e7ait comme charg\u00e9e de recherche. En septembre 2023, elle a d\u00e9but\u00e9 sa sp\u00e9cialisation en psychoth\u00e9rapie syst\u00e9mique. Elle pratique \u00e0 pr\u00e9sent en tant que psychoth\u00e9rapeute dans le service du CHUV o\u00f9 elle a effectu\u00e9 son travail doctoral.<\/em><\/p>\n\n\n\n<p>Le programme de r\u00e9habilitation vocationnel le plus efficace est l\u2019<em>Individual Placement and Support<\/em> (IPS). Toutefois, peu d\u2019\u00e9tudes se sont pench\u00e9es sur son efficacit\u00e9 pour les troubles de la personnalit\u00e9 (TP). Les personnes concern\u00e9es pr\u00e9sentent des difficult\u00e9s engendrant des probl\u00e8mes importants sur le lieu de travail et pourrait constituer un obstacle pour les sp\u00e9cialistes IPS. Cette th\u00e8se vise l\u2019impl\u00e9mentation d\u2019une nouvelle approche dans le dispositif IPS afin d\u2019am\u00e9liorer la prise en charge des participants au programme pr\u00e9sentant un TP.<\/p>\n\n\n\n<p>Nous avons d\u2019abord compar\u00e9 l\u2019efficience d\u2019IPS dans le canton de Vaud, en Suisse, pour les personnes avec ou sans TP. L\u2019efficacit\u00e9 d\u2019IPS \u00e9tait effectivement diminu\u00e9e pour les personnes pr\u00e9sentant un TP. Puis, nous avons explor\u00e9 les difficult\u00e9s rencontr\u00e9es par les sp\u00e9cialistes IPS dans leur pratique. Ils\u00b7elles reportaient plus de difficult\u00e9s dans le suivi des personnes pr\u00e9sentant un TP. Ceci am\u00e8ne \u00e0 la proposition d\u2019une formation des sp\u00e9cialistes IPS au <em>Good Psychiatric Management<\/em> pour le TP borderline afin d\u2019en am\u00e9liorer la prise en charge. D\u2019une part, l\u2019utilit\u00e9 de cette nouvelle modalit\u00e9 a \u00e9t\u00e9 \u00e9valu\u00e9e aupr\u00e8s des sp\u00e9cialistes IPS. Une \u00e9tude qualitative a \u00e9t\u00e9 conduite, afin de voir dans quelle mesure cette addition a conduit \u00e0 des changements de pratiques, et comment ces modifications \u00e9taient per\u00e7ues. D\u2019autre part, l\u2019int\u00e9r\u00eat et l\u2019efficacit\u00e9 de l\u2019impl\u00e9mentation ont \u00e9t\u00e9 mesur\u00e9s qualitativement et quantitativement aupr\u00e8s des client\u00b7e\u00b7s concern\u00e9s dans une \u00e9tude de cas multiples. Les sp\u00e9cialistes IPS jugeaient de mani\u00e8re positive l\u2019int\u00e9gration du <em>Good Psychiatric Management<\/em> dans leurs pratiques. Les r\u00e9sultats des client\u00b7e\u00b7s \u00e9taient prometteurs\u00a0: ils\u00b7elles exprimaient \u00eatre satisfait\u00b7e\u00b7s de leurs suivis et recommanderaient une telle prise en charge \u00e0 d\u2019autres personnes.<\/p>\n\n\n\n<p>Les travaux pr\u00e9sent\u00e9s dans cette th\u00e8se soutiennent l\u2019am\u00e9lioration des pratiques des sp\u00e9cialistes IPS.
